startThis presentation by Franklin County Home Health Agency staff will give you information on your end-of-life options and how to start what can be a difficult conversation with your loved ones. “Start the Conversation” is a program whi       ch encourages people to learn about their end-of-life care choices, talk about their wishes with loved ones and doctors and put those plans into writing.Participants will learn about the Advance Directive for Health Care and will walk through choosing a Health Care Agent (the person you designate to make health care decisions for you when you are unable to make decisions for yourself) and possible treatment plans and hypothetical medical situations along with other resources. This program is designed to help you understand and think through topics relevant to end-of-life care.Please RSVP to Nancy 393-6721
